  • Program 2:

  • Description: Intensive Case Management Services include an evaluation of an individual’s strengths and needs, Goal setting and service planning, Linkage with support services, Advocacy for individuals rights to services, Pregnant women and women with children services, Increasing treatment outreach, Providing assessments and making appropriate referrals (employment, child care, housing, etc.), efficient and effective utilization of community resources, orienting clients to treatment, increased retention of clients in treatment, supporting treatment compliance, facilitating access to additional community and treatment services, monitors referrals and provides follow-up checks, teaches specific process on how to set goals and objectives in any of client life areas, and advocacy.

  • Program 3:

  • Description: The Tobacco Cessation is a Worksite, One on One, Group, or phone sessions depending on individual’s preference. Sessions are typically 30 minutes to 1 hour in length. Free $100 voucher with participation and completion of program to be used for NRT or prescription pharmaceutical. (18 and over)
    Target population: Individual wanted to quit tobacco use

  • Description: Too Good for Drugs is a school-based prevention program designed to reduce risk factors and enhance protective factors related to alcohol, tobacco and other drug (ATOD) use among students

    Age Range: Elementary/Middle school age
